Grant Evaluations are required for all grants awarded by the Missaukee Area Community Foundation.

All Grant Programs may use the Common Grant Evaluation Form which can be accessed by clicking here.

Once you have completed your Grant Evaluation you may email it to the Foundation at cacf@cadillacfoundation.org.
